Strengthen Capacity of the South African Government’s Department of Health to Provide Quality and Sustainable Clinical Care for HIV-Infected Patients with Complicated HIV and HIV/TB Treatment Management, Including 2nd and 3rd Line and Other Antiretroviral Therapy under the President’s Emergency Plan for AIDS Relief (PEPFAR)

Grant Amount: Approximate Current Fiscal Year Funding: $5,000,000; Approximate Total Project Period Funding: $50,000,000.

The goal and objectives of the program in South Africa are to support the South African Government Department of Health (SAG DOH) to provide effective clinical management of adult and pediatric patients with complicated HIV and HIV/TB treatment needs, including 2nd/3rd line and other alternative ART, through funding and capacitating regional Centers for Advanced Clinical Management of HIV/AIDS. These centers will provide high quality referral or tertiary-level HIV and HIV/TB management services, and support the development of sustainable capacity for effective management of the same patients at district level DOH facilities.

Support will be limited to three provinces (Kwa-Zulu Natal, Free State, and Eastern Cape) for the first two years of the program, but may subsequently be expanded to additional provinces based on indentified need and availability of funding.

Objective 1: Establish and/or strengthen at least one Regional Center for Advanced Clinical Management of HIV/AIDS in each province, to provide clinical management for adult and pediatric patients with complicated disease presentations and/or with complicated HIV and HIV/TB treatment management, including 2nd/3rd line and other alternative ART

Objective 2: Establish adequate up-referral systems to ensure relevant and timely access to regional Centers for Advanced Clinical Management of HIV/AIDS, as well as down-referral of stabilized patients to capable district level DOH facilities.

Objective 3: Build and support the capacity of DOH facilities at district level for the management of adult and pediatric patients with complicated disease presentations and/or with complicated HIV or HIV/TB treatment management, including 2nd/3rd line or other alternative ART. (This objective will be achieved in close collaboration with other PEPFAR district-based care and treatment support partners).

Objective 4: Engage with SAG DOH to plan for timely and full transition of all aspects of the advanced HIV/AIDS clinical management services to the SAG DOH by the end of the project period.
